INTRODUZIONE

Le fratture prossimali dell’omero raramente sono esposte, rappresentano il 6% delle fratture dell’adulto, aumentano con l’età e affliggono soprattutto le donne ^1,2 (nell’87% dei casi per caduta dalla posizione eretta). La classificazione più utilizzata è quella proposta da Neer ³ma quella dell’AO, che considera la vascolarizzazione dei frammenti, si addice meglio al nostro argomento^4,5. Nello specifico ci occupiamo delle “C” ovvero quelle scomposte e con probabile compromissione vascolare⁶. Il riassorbimento delle tuberosità nelle endoprotesi di spalla su frattura rappresenta una delle cause di insuccesso di questa soluzione chirurgica⁷. I pazienti lamentano dolore e grave impedimento funzionale. La soluzione è la revisione chirurgica con una protesi inversa, oggi più facile, veloce e meno invasiva, grazie ai nuovi impianti modulari.

ANAMNESI ED ESAME OBIETTIVO
La paziente, donna di 73 anni, giunge all’osservazione a seguito di trauma per caduta da posizione ortostatica sulla spalla sinistra (arto non dominante) nell’ottobre 2009. Alla radiografia eseguita in PS si evidenzia una frattura a 4 frammenti secondo Neer ³ o 11C3⁵ secondo l’AO (fig. 1). Sottoposta ad intervento chirurgico di endoprotesi di spalla 4 giorni dopo l’evento traumatico, esita dolore continuo con esacerbazione notturna ed impedimento funzionale grave. L’esame obiettivo (Arom: active range of motion) a un mese è il seguente: elevazione anteriore (EA) 40°, abduzione (ABD) 40°, extrarotazione al fianco (Er1) 15°, intrarotazione al fianco (Ir1) gluteo. Il constant score è 14/100. A 6 mesi l’obiettività è la seguente: (Arom) EA 60°, ABD 40°, Er1 10°, Ir1 gluteo con dolore continuo. DIAGNOSI

TRATTAMENTO
Nell’aprile 2011 viene sottoposta a revisione della protesi mediante protesi inversa.
La paziente viene posizionata in decubito semisupino, l’incisione chirurgica segue la precedente via delto-pettorale. Si evidenzia la completa assenza delle tuberosità nonostante la presenza dei fili di sutura predisposti alla sintesi. Il tendine del sottoscapolare è di qualità discreta mentre non sono recuperabili il sopraspinoso ed il sottospinoso. Purtroppo il sistema precedentemente impiantato non prevedeva la conversione ad inversa, si procede quindi alla rimozione dello stelo omerale fissurando l’omero prossimale.
